For people on a GLP-1, TRT or HRT

Every dose changes something.
Your doctor sees you twice a year.

GLP-1s, testosterone and HRT are the most powerful drugs most people will ever take on their own. ILAI correlates your labs, your watch, your meals and every dose, every day, reasons through what changed, and tells you what is working, what is not, and what to do about it before it becomes a problem.

ILAI's midday check on Alex's phone: steps behind pace, 1,550 kcal and 161 g protein so far, HRV 27% under usual, and overnight blood oxygen worth raising with the clinician
Tuesday, 2 pm. ILAI’s midday check on Alex’s phone, as written.

What you tell ILAI

Tell it anything. It remembers everything.

A meal, a question, a symptom, a blood donation, a workout your watch missed. One sentence or a photo. ILAI puts it on your record and re-analyzes your whole day.

  • Sarahturkey and avocado wrap with baby carrots for lunch, only got through half the wrap
  • MichaelCan I go up to 0.5 mg of semaglutide next week?
  • AlexI actually did strength training for over an hour today, I just forgot to start the workout on my watch

Your lab panel

“In range” is not the same as fine.

Reference ranges are built for the average person, not someone on testosterone or a GLP-1. Alex’s hematocrit came back in range. ILAI saw it was climbing, close to his ceiling, and drawn eleven days after a blood donation, which makes it measure low. The real number was likely higher. A lab report can’t infer that. ILAI did, the afternoon the results landed.

Three spotlight rows after the Sep 23 draw: hematocrit 50.44% in range and worsening, HDL 28.5 low, down 27% from the prior draw, ApoB 77 in range, down 21%The hematocrit watch note: 50.44% on Sep 23, ceiling 52; donated Sep 12, before this draw; recheck in 4 to 6 weeks; donate on schedule if still at or above 50

Your ring or watch

Your HRV dropped. Do you know why?

Your watch can tell you the number. It can’t tell you it fell the week your dose changed, the night you didn’t sleep, the day your stomach turned. ILAI holds all of it, so a bad night stays a bad night, and a real trend gets named. Apple Watch, Oura or Whoop.

“The 7-day averages look strong, but the most recent single readings (resting HR 73.0, HRV 24.9) disagree with them. A sleepless night, fluid lost to diarrhea or a one-day watch artifact could each explain the gap.”ILAI, the daily analysis
Three signal tiles: Sleep 7.04 h, up 13% on the baseline; resting heart rate 63.4, down 7.7%; HRV 39.7 ms, up 17.9%, each with its seven-day line
The week’s signals on Alex’s record, each against its own baseline.

Your prescription

Thursday’s dose. Should you take it?

Four days after restarting tirzepatide, Alex’s stomach turned. No appointment for weeks. ILAI laid out what could explain it, made Thursday’s dose the decision point, and told him exactly what to track and when to call his prescriber. It never prescribes. It makes sure you never decide blind.

“Several explanations fit the diarrhea, and the record can’t separate them yet: the tirzepatide restart (2.5 mg since Oct 1, 2026), an IBD flare, something you ate. The next tirzepatide dose is Thursday, so how today goes decides that call.”ILAI, the daily analysis
Alex’s active protocol: testosterone cypionate 40 mg (120 mg a week), HCG and NAD+, and tirzepatide 2.5 mg restarted at the starting dose; week 3 of 8 with two reviews pending
Alex’s protocol: tirzepatide restarted at 2.5 mg beside his TRT.

Your meals

Lose the fat. Keep the muscle.

On a GLP-1 your appetite goes, and protein goes with it. Weight comes off, and some of it is the muscle you worked for. ILAI sets a protein floor for your body and your goal, counts every meal you describe in a sentence or a photo, and tells you at 2 pm, not next month, how far you are from it.

“Turkey and avocado wrap, ½ wrap (left the other half): 245 kcal, 12 g protein. Protein 54 g logged so far; the floor set for you is 125 g.”ILAI, to Sarah, after “only got through half the wrap”
The nutrition card: 1,550 kcal of a 2,400 ceiling, 850 left; protein 161 g, floor met, 17 g to the 178 g target; fluids 1.5 L of 2.9; steps 4,191 behind pace; Zone 2 session due
Alex’s day at 2 pm, against his ceiling and floors.

Your photos

The scale can’t tell fat from muscle. Your photos can.

Every two weeks, four poses. ILAI compares each one to the last and tells you where the change came from, region by region, correlated with your weight and your labs. Your photos are seen by you only.

The photo check-in card: four poses (front, flexed, side, back) with the head out of frame, New check-in and Compare
Alex’s check-ins, face out of frame. Private to you.

Also this week

Nobody asked. ILAI caught it anyway.

Michael · on a GLP-1

“Can I go up to 0.5 mg of semaglutide next week?”

Your record shows you’re already on 0.5 mg of semaglutide, and the next increase should wait for a lipase result. Your week 4 titration review had a safety rule: hold the increase if lipase is above 60 U/L. Lipase has never been measured.
Sarah · on HRT

An ECG from last month, and a medication on her list.

Your QTc was 482 ms on Sep 20, 2026, and Escitalopram is on your medication list. Escitalopram can lengthen the QT interval. Share this with the prescriber.
Alex · on TRT

A draw eleven days after a blood donation.

Hematocrit at 50.44% came 11 days after your Sep 12, 2026 donation with ferritin now 124 ng/mL, so it likely understates your settled level. On a similar 120 to 130 mg/week dose on Feb 18, 2026, hematocrit read 50.7%.

How it works

Set up once. ILAI works every day after.

  1. Bring what you have.Lab reports from any lab, as a PDF or a photo. Your medications and doses. Apple Health from the iPhone app, or Oura and Whoop from your browser.
  2. ILAI analyzes it every day.Meals, sleep, heart rate, steps and doses, against your plan and your labs. A dose step waits when a lab says it should.
  3. You hear what matters.A morning brief, a midday check and an evening recap. What changed, what it means, and the one thing to do today.
